AGREEMENT WORK ORDER FINANCIAL PROJECT ID:
413615-8-78-05 BETWEEN THE CITY OF EDGEWATER
AND THE STATE OF FLORIDA, DEPARTMENT OF
TRANSPORTATION ("DEPARTMENT"); REPEALING
ALL RESOLUTIONS IN CONFLICT HEREWITH AND
PROVIDING FOR AN EFFECTIVE DATE.
WHEREAS, the City of Edgewater and the Florida Department of Transportation (FDOT)
would like to enter into a State Highway Lighting, Maintenance, and Compensation Agreement
Work Order (FY 21/22), Contract # AN -869 for Financial Project ID 413615-8-78-05 (FEID #VF -
596000314 -002).
WHEREAS, FDOT has requested the City of Edgewater execute and deliver the
Authorization Letter/Work Order and the State Highway Lighting, Maintenance and Compensation
Agreement Certification for the aforementioned project, #413615-8-78-05.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of
Edgewater, Florida:
Section 1. The City Council of the City of Edgewater hereby approves entering into the
State Highway Lighting, Maintenance and Compensation Agreement — Financial Project
#413615-8-78-05, Contract #AN -869 (which is attached hereto and incorporated herein as
Exhibit "A") and authorizes the City Manager to execute same.
Section 2. All resolutions or parts of resolutions in conflict herewith are hereby repealed.
Section 3. If any portion of this resolution is for any reason held or declared to be
unconstitutional, inoperative, or void, such holding shall not affect the remaining portions of this
resolution. If this resolution or any provisions thereof shall be held to be inapplicable to any
person, property, or circumstances, such holding shall not affect its applicability to any other
person, property, or circumstance.

Section 4. This Resolution shall take effect upon adoption.

You are hereby authorized to perform maintenance of the State Highway Lighting Systems within your
jurisdiction in accordance with the terms and condition-, of the State Highway Lighting, Maintenance, and
Compensation Agreement.
The total compensation for Fiscal Year 21/22 paid Linder this Work Order is $43,119.45. This amount
reflects a 3% increase per unit cost from the previous year. The compensation period for this work- order
will begin from July 1, 2021 and expire June 30, 211,22.

If you wish to receive payment for the Fiscal Year 20/21 Work Order, please submit an invoice to
this office any time after May 19, 2021. Invoices for Fiscal Year 20/21 must be received no later than
December 19. 2021. Per the State Highway Lighting, Maintenance, and Compensation Agreement
paragraph 2.c. which states: "Invoices may be submitted any time after May 19" of the fiscal year in
which the services were provided, but no later than 180 days after the end of the fiscal year. Payment
shall be made in one Jump sum, as provided in paragraph 4 hereof,

sample invoice is included on your invoice Submission.
