alarms can be registered at no charge provided the application is submitted
within 30 days from the installation date.
Pursuant to Edgewater Resolution No. 94-R-05, you are hereby assessed a fee
of $25.00. If this is a new installation, please provide the City Clerk's office with
your alarm permit application and a copy of the installation certificate. If the
payment is not received within thirty days of the date of this letter, a lien will be
recorded. Payment may be mailed to the City Clerk, P.O. Box 100 Edgewater,
FL 32132-0100 or delivered to 104 N. Riverside Drive. The cut off portion of the

NARRATIVE
On the above date and time,I responded to Diamond Precision Machining located at 137 West Marion Avenue,Unit#2 in reference to air
alarm.
Upon arrival,I made contact with the owner of the building, Shirley Boyce who stated she opened up the unit of her tenant but did not
know he had an alarm. Shirley said she was checking to see if the water was still working since her contractor cut the line.
Everything was okay at the building.
This report is for documentation.
