above-listed address, the City became aware of failure to acquire an alarm permit, therefore, the
amount of fifty and 00/1 00 dollars ($50.00) plus any recording fees, foreclosure costs, attorney's fees
and simple interest in the amount of eight (8) percent per annum from the date this lien is filed. Such
lien shall be prior to all other liens on such property except the lien of state, county, and municipal
taxes and shall be on a parity with the lien of such state, county and municipal taxes.

registered with the City. The cost to register an existing alarm is $50.00. All new
alarms can be registered at no charge provided the application is submitted
within 30 days from the installation date.
Pursuant to Edgewater Resolution No. 94-R-05, you are hereby assessed a fee
of $50.00. If this Is a new installation, please provide the City Clerk's office with
your alarm permit application and a copy of the installation certificate. If the
payment is not received within thirty days of the date of this letter, a lien will be
recorded. Payment may be mailed to the City Clerk, P.O. Box 100 Edgewater,
FL 32132-0100 or delivered to 104 N. Riverside Drive. The cut off portion of the
letter below must be included with the payment. If you have any questions,

If you believe the alarm was not an alarm as defined in Section 12-61 Edgewater
Code of Ordinances, you may appeal that decision to the Citizens Code

NARRATIVE
On November 22,2008 at 2:01 P.M. I was dispatched to 2740 Date Palm Drive,in regards to a residential alarm.Operator 509 from
Royal Electronics advised the Regional Communication Center a front door alarm had been activated.
I arrived and made contact with Nancy Garvin,who stated the residence belonged to her Daughter,Lashonda Miller.Nancy advised
me Lashonda was out of town and she was watching the house. Nancy said she accidentally activated the alarm and did not know how to
turn it off.
The Regional Communication Center advised me Royal Electronics contacted them again and stated Nancy was allowed to be at the
residence. I confirmed Nancy's identification using a Florida driver's license.
There was no alarm permit on the residence.This report was not weather related.
No further information.
