The record owner is MARK F. WORTHINGTON, 39 Kenwood Road, Wethersfield, CT 06109. As
a result of a false alarm that occurred on June 11, 2009 at the above-listed address, the City became
aware offailure to acquire an alarm permit, therefore, the amount of fifty and 00/1 00 dollars ($50.00)
plus any recording fees, foreclosure costs, attorney's fees and simple interest in the amount of eight
(8) percent per annum from the date this lien is filed. Such lien shall be prior to all other liens on
such property except the lien of state, county, and municipal taxes and shall be on a parity with the
lien of such state, county and municipal taxes.

You are hereby notified that to have an alarm system in Edgewater it must be
registered with the City. The cost to register an existing alarm is $50.00. All new
alarms can be registered at no charge provided the application is submitted
within 30 days from the installation date.
Pursuant to Edgewater Resolution No. 94-R-05, you are hereby assessed a fee
of $50.00. If this is a new installation, please provide the City Clerk's office with
your alarm permit application and a copy of the installation certificate. If the
payment is not received within thirty days of the date of this letter, a lien will be
recorded. Payment may be mailed to the City Clerk, P.O. Box 100 Edgewater,
FL 32132-0100 or delivered to 104 N. Riverside Drive. The cut off portion of the
letter below must be included with the payment. If you have any questions,
please call (386) 424-2400 Extension 1102.
If you believe the alarm was not an alarm as defined in Section 12-61 Edgewater
Code of Ordinances, you may appeal that decision to the Citizens Code
Enforcement Board, (386)424-2414, pursuant to Section 12-67, Edgewater Code
of Ordinances.

NARRATIVE
I was dispatched to 817 Star Reef Ln after Guardian Security(Operator 120)reported a residential alarm.
Contact was made with Melissa Ropp,who arrived on scene as 1 arrived. Melissa Ropp remained outside while 1 conducted an interior
check of the residence. Nothing suspicious was observed. Melissa Ropp entered the residence and advised she saw nothing out of place.
Melissa Ropp reset the alarm system and secured the home.
No inclement weather. No visible alarm permit. This alarm was false. Documentation only.
