1. On Sheet CO -01, notes 20 and 21, remove references to "seeding ".

Landscaping
1. Indicate sizes of shrubs and trees in accordance with Sections 21 -54.02 (e) and
21 -21 -55/03 (b).

2. Add a note indicating compliance with Section 21 -54.02 (c).

3. A total of 187 shrubs are required (with a minimum of 38 in the landscape buffer).
Several of the proposed shrubs have been determined to be groundcover. Verify
acceptable shrub /tree types with City staff.

palmettos are not acceptable trees.
Building Department
1. Construction drawings accepted after October 1, 2005 shall comply with the 2004
Florida Building Codes for 120 MPH wind speed within the impact debris region.

2. Provide a letter from the engineer advising that the existing footings are sufficient
to accept the additional load of the renovation to the existing building.

Fire - Rescue Department
1. The required fire flow for this project is 500 gallons per minute.

2. The location (and size of water main) of the closest fire hydrant must be indicated
on the plans.

Vehicle /pedestrian circulation
1. A copy of the FDOT permit for the driveway connection and improvements must
be provided.

plan.
Department of Environmental Services
1. According to the survey, the site elevations are assumed. It does not appear that
the "assumed" elevations are correct and they should be re- evaluated for accurate
construction.

2. With no positive outfall, stormwater design is to be for a 100 - year /3 day storm
event in accordance with Section 21 -53.04 (k).

requirements.
3. Areas on the north and west side of the existing building shall be sloped or swaled
to allow drainage to the retention areas. Please put flow arrows on the grading
plan for clarification.

4. Dumpster enclosure detail needs to be shown on the plans (three sides only — no
gate required on the west facing side until such time a dumpster is installed).

5. Commercial water connection in the City of Edgewater are required to install a
reduced pressure backflow prevention device on the downstream side of the water
meter but prior to the first branch connection. The City of Edgewater Standard
Construction Details includes a specific detail for this application. The backflow
prevention device must be on the approved list of devices available at the
Department of Environmental Services Administration Office located at 115 East
Park Avenue or is verifiable by telephone by calling 386 - 424 -2460.

6. On sheet C2 -02:
a. Best management practices — the first paragraph should reference the City
of Edgewater.

County.

Legal Department
1. The Title Insurance Policy /O &E reflects the following:
a. The Commitment is dated July 12, 2005 but the proposed insured is
Riverside National Bank, not Florida Casa Net (as the owner). However,
the Policy is dated January 31, 2005 with the insured as Florida Casa Net.
One or the other should be modified to the correct date or owner.

b. The Commitment reflects two easements to the City of Edgewater located
in OR 641/PG 584 and recorded in OR 669/PG 724 and OR 669/PG 710 —
these items must be provided.
