General 1. The square footage of the buildings on the site plan and building plan are not consistent. Please revise. OK 2. Provide a plan/profile of the bank building to verify compliance with height requirements. OK 3. Provide details of proposed signage. OK 4. Clearly indicate dumpster location on site plan. Indicate dumpster location for the bank as well. WILL LABEL 5. The methodology proposed to be used to compile the traffic study is acceptable. Please provide copy of completed traffic study. STUDY ON ORDER 6. Provide details regarding the possible division of the parcel. A separate parcel for the bank may affect parking and landscaping requirements. OK 7. Revise gate detail on dumpster enclosure to indicate stockade fencing material. OK 8. Right and left hand turn lanes, constructed in accordance with FDOT standards, are required in accordance with Section 21 -52.05 (e) (g). WILL APPLY WITH FDOT 9. Indicate compliance with Section 21 -52.03 with particular emphasis on bypass lane and exit turning radius. OK 2. Indicate location of adjacent driveway (Bank of America). OK 3. Indicate location of driveways on east side of US 1 for compliance with Section 21 -52.02 (g). OK 4. A joint use driveway easement /agreement may be necessary. WILL APPLYFOR MSD TO INCLUDE JOINT USE DRIVEWAYAGREEMENT 5. Aisle width in the rear of the retail building is not in compliance with Section 21- 57, Table V -7. Indicate compliance on revised submission. * JILL WILL CHECK WITH DARREN ON THIS COMMENT 6. Loading berths are not in compliance with Section 21- 57.05, Table V -9. Indicate compliance on revised submission. WILL REDUCE "REAR ENTRANCEWAY" FROM 5' TO 3'. THIS WILL ADDRESS THIS COMMENT 7. "One way /do not enter" signage should be provided for the bank drive up aisles and the rear drive behind the main building. OK 8. Provide detail of raised curb. OK Landscaping 1. Provide details of compliance with Volusia County tree protection standards (15% of existing trees must remain on site). *JILL WILL CHECK WITH DARREN. THERE IS ONLY ONE SPECMEN TREE ON SITE (35 " OAK) WHICH IS IN THE POPOSED DRIVE THR U LANE. 2. Provide a note that irrigation for landscaping will meet the requirements of the Volusia County "water wise" ordinance. OK 3. Revise tree and shrub sizes in accordance with Section 21 -54.02 and 21- 55.03. OK 4. Per Section 21- 55.07, 80% of the specimen trees on site must be protected. OK 5. Provide alternative replacement tree for cabbage palms. OK —NO CABBAGE PALMS PERMITTED 6. A total of 131 trees are required yet only 77 are shown. *JILL WILL CHECK WITH DARREN. THE APPLICANTS ARE PROPOSING UPGRADING THE CALIPER OF THE TREES IN LIEU OF ADDING MORE TREES. 7. Indicate type of sod to be used in area to the rear of the bank. OK SP 0520— Middletown Plaza Technical Review Committee Comments For original submittal December 29, 2005 8. A total of 233 shrubs and 10 trees are required in the buffer yard to the rear of the retail building. Plan is not clear and compliance with this requirement cannot be verified. OK ON engineer (David King — Ouentin L. Hampton, Associates, Inc. 1. Water main connection to the fire hydrant should be 6" in diameter minimum (2" is shown). OK 2. Water meters should be placed at connection to 6" water main. OK 3. Easement(s) is /are required over any on -site water /sewer facilities to be maintained by the City. OK 4. Proof of legal ability to connect to off -site sewer should be provided. PER TERRY, APPLICANT WILL NEED EASEMENT TO ACCESS SEWER. 5. A FDEP permit is required for the sewer construction. OK 6. If City is to own/maintain on -site sewer, shallow type manholes per City design standards will be required. OK 7. A copy of the required FDOT driveway connection permit should be provided to the City. OK 8. Sidewalks are required per Section 21- 52.04. OK 9. A copy of the required SJRWMD stormwater permit should be provided to the City. OK 10. The applicant should justify the percolation rates used in the pre /post discharge. OK 11. The overflow should be connected to public facilities (FDOT US 1) per Section 21- 53.06. OK 12. NPDES permit coverage is required. A copy of the NOI should be provided to the City. OK 13. Site lighting should be designed so not to impact adjacent properties. OK Department of Environmental Services Water and sewer 1. The site plan indicates a 2" x 6" tapping sleeve and valve at US 1 to supply potable water for domestic and fire flow applications. It should 6" x 6" tapping sleeve and valve. OK SP 0520— Middletown Plaza Technical Review Committee Comments For original submittal December 29, 2005 2. Section 21- 250.03 (c) (4) of the City's Land Development Code calls for a 12" water main capable of delivering 2,500 GPM for a commercial area in excess of 10,000 square feet. It is understood this is infill development and the developer has no reasonable control over the size of the water lines serving this project. However, to optimize fire flow and water quality for this project and the general area, the following actions are proposed: a. The fire line supplying water to the fire hydrant located within the project should be upgraded to 8 ". full CI" b. There is a 6" water line stubbed out from Mariner's Gate on the eastern end of the occupied portion of Pine Avenue. The City would like to extend this line and tie it into Middletown Plaza to join the proposed fire line there. By providing the two separate sources 6" water line feeds, the fire flow will be greatly enhanced and the redundancy of supply would help ensure uninterrupted water service to Middletown Plaza. The City will supply labor and materials for this portion of the water line (Pine Avenue to the proposed hydrant location). The developer will provide engineering design, permits (DOH) and the necessary easements to facilitate construction of the water line. 3. The backflow prevention devices on the downstream side of the two separate 2" water meters are both required to be reduced pressure devices on the list of approved devices available from the DOES Administration office at 115 E. Park Avenue or verify by phone at 424 -2460. OK 4. The sewer line from Middletown Plaza appears to cross private property to tie into the existing manhole near the northwest corner of the project. An easement should be acquired by the developer for this portion of the sewer line. OK 5. It is unclear, at this point, as to the legal status of the section of vehicular traveled area between Pine Avenue and US 1 which runs through this project's boundaries in the southeast section. Please clarify. *JILL WILL CHECK WITH LEGAL. 6. The ownership of the main sewer line into which the flow from this project will be entering is being researched to ensure it is in City control and ownership. OK Stormwater 1. Comments regarding the stormwater calculations are pending. OK OK 2. A FDEP NPDES construction permit will be required. 3. A Florida DOT permit will be required. SP 0520— Middletown Plaza Technical Review Committee Comments For original submittal December 29, 2005 4. Overflow weir must go to a dedicated stormwater system. Stormwater is not permitted to flow onto private property. OK 5. Sidewalks should be 3,000 psi concrete cement. OK 6. Fire hydrants are to be Clow Medallion or Mueller A -423 only. OK 7. To further clarify water and sewer comment #1 from above, the fire line connection should be separate from the buildings' service connections. OK 8. Thrust blocks are not permitted. OK 9. Unions are required on backflows. Backflows should be an approved reduced pressure device. OK 10. Sewer lines are required to be SDR 26, ASTM D -3034. OK Building Department 1. Construction drawings shall comply with all 2004 Florida Building Codes for 120 MPH wind speed within the impact debris region. OK 2. Tenant separations will be 1 -hour separation from floor to ceiling. OK 3. Fire suppression systems may be required depending on type of tenants. OK Legal Department 1. Provide proof that the person who executed the application has the authority to execute on behalf of Federal Trust Bank. OK Fire - Rescue Department 1. An additional hydrant is required at the US 1 entrance of the site in accordance with Section 21- 250.04 (c). OK 2. The required fire flow for the bank is 750 gallons per minute. The required fire flow for the retail building is 1000 gallons per minute. This building will require the installation of a fire sprinkler system. BANK DOES NOT REQUIRE SPRINKLERS
