OK General 1. A joint use easement agreement will be required for driveway access. OK 2. Indicate outfall to a public conveyance. OK 3. Indicate if concrete pad located in the southwestern portion of parcel is proposed or existing and proposed use. OK 4. Correct zoning classification on Sheet 1. OK 5. Provide copy of threatened /endangered species report. OK 6. Indicate flood zone lines on site plan. OK 7. Provide copy of easement agreement for driveway access. OK Landscaping 8. No tree survey has been provided. Provide a tree survey and indicate which trees are to remain and which are to be removed. OK 9. Landscape plans must indicate from Section 21-54.02(c) "All plant materials used in conformance with the requirements of this Section shall be Florida grade #1, as established, and periodically revised by the Florida Department of Agriculture and Consumer Affairs." OK 10. Compliance with minimum tree protection standards per Section 21.55. OK 11. Based on proposed Phase I development, the proposed landscaping tree and shrub numbers are significantly deficient. There is no notarized Authorization of Owner from Dover Properties giving Phil Beede authority to act on owner's behalf. OK 2. There is no O &E/Title Insurance or Title Report submitted. The ingress /egress easement will need to be dedicated as a utility casement also for maintenance of the six-inch water line. See above 4. The water meter needs to be at the property line with a reduced pressure backflow device. OK 5. There is only a two (2 ") water line along the front of the property on the east side of Mango Tree Drive. The six inch line proposed will need to be extended from approximately 200 feet from the north on Lime Tree Drive. The six-inch line ends at the last house from Woodrick Subdivision. In addition, it is recommended to extend the six-inch water line south under the driveway to prevent any damage to the driveway at a later date for extension of this water line. OK 6. Per the Land Development Code 21-53.04(k.), post development rates and volumes must be designed for the 100-year storm in land locked basins. OK 7. Per the Land Development Code 21 a dumpster with an enclosure �h�ll be required, if roll out trash removal is used, can a pad can be provided, noted as a future' dcr:torpad, whew enclosure? No water or wastewater capacity allocations will be allotted for any development until such time as all impact fees are paid. OK Engineer - David King, P.E. — Quentin L. Hampton Associates, Inc. 1. Applicant should provide documentation demonstrating that the area of the site not included in calculations does not drain to the area of project. OK 2. Applicant to correct pervious /imperious areas tabulation. Please provide a complete tabulation of existing and proposed impervious areas separating buildings, sidewalks, pavement, concrete slabs etc. OK 3. A SJRWMD permit is required and should be provided to the City. OK 4. Dump ster/e nclosure should be provided per Section 21-34.08. See ES #7 5. Roofs drainage should be routed to the stormwater treatment system. OK 6. For site development review purposed, all building floor plans should be submitted depicting, doorways, any change of level, building height, etc. OK 7. VCHD permit for the water improvements is required. OK 8. NPDES permit coverage should be obtained as required. OK 9. FDOT stop sign and stop bar should be provided at driveway connection with Mango Tree Drive as well as at intersection of driveway isles. Pavement demarcation should include circulation arrows and signs, and loading area striping. OK 10. Stormwater system design /calculations should be provided for the proposed 1,340 LF access driveway off Mango Tree. OK 11. Dead-end water main exceeds maximum length allowed of 250 LF for a 6" diameter pipe per Section 21 -.1.. ANWWWANWA" OR 12. Landscape plan shows plants blocking access driveway. Applicant to correct. OK 13. A geotechnical investigation report should be provided and used in the stormwater calculations. OK 14. A threatened and endangered species investigation report should be provided. OK 15. More detail regarding the stormwater pond discharge should be provided. Permitting through FEC may be required. OK 16. VCHD septic tank permit may be required.
